Each form has a different number – do NOT use a form photocopied
from someone else. If you apply through the Internet or download an
application form, your CAO number will appear on the screen. Make
sure that you enclose all the documents you have available at the time of
your application. Send certified copies and NOT the originals. We cannot
be held responsible for your documents’ safekeeping. Tick the checklist
on the form to show us what you are enclosing.
• A certified copy of your identity document or passport:
If you do not already have an ID book, apply for one now – you will
need it when you start studying.
• A certified copy of your NSC, NC(V), Senior Certificate
(if already issued):
If you have the results but not the certificate, send us a copy of the
Statement of Results/Symbols. If you are writing NSC in 2016, DO
NOT send us your final results unless we ask you for them.We receive
them directly from the Department of Education.
• A certified copy of your school examination results:
If you are still at school, we need a copy of your Final Grade 11, June
Grade 12 and Trials results (or any of these that are available).
Institutions use these results tomake early selection decisions. Please
ensure that your name, surname and ID number appear on the same
page as your results.
• A certified copy of your school-leaving examination results:
If you havewritten foreign or international school-leaving examinations,
please send us copies of your results.
• A copy of your academic record and certificate of good conduct:
If you have already studied at a post school institution.
• Residence or Study Permit:
If you are not a South African citizen and do not have a South African
ID, we need a copy of any Residence or Study Permit you have been
issued. When documents (e.g. examination results) become available
after you have submitted your application, please send them to us,
clearly marked with your CAO number. If you do not have any of
these documents, then you must still submit your application and pay
the application fee. You may send the documents later.

Things to watch out for when filling in the application form
PLEASE TAKE NOTE
• Do NOT use a photocopy of the application form. Each form contains
a unique CAO number.
• If you wish to make changes later in the year (e.g. changes to contact
information, programme choices, etc., complete the necessary
Change of Mind or Change of Contact form – email, fax or post ONLY
the changes to us – DO NOT complete a new application form.
• Do NOT send us more than one application form or complete more
than one online application for the same entry year as it will be treated
as a duplicate application and will not be processed until you confirm
your final list of choices.
