5. How to Apply Through the CAO

• Get copies of documentation. See page 44 & 45 and the checklist on the
application form.
• Arrange payment (page 48 and 49).
• Staple documentation and proof of payment to the form or declaration.
• Post, fax or email everything to us OR hand it in at the CAO offices (page 4).
• Keep your receipt and make a note of your CAO number and EasyPay
number.
Print out the declaration, consent form and payment details.
Capture your application on
the CAO website (page 40).
