Make sure that you enclose all the documents you have available at the
time of your application. Send certified copies and NOT the originals.
We cannot be held responsible for your documents’ safe keeping. Tick
the checklist on the form to show us what you are enclosing.
A *True Certified Copy of your identity document or passport:
If you do not already have an ID book, apply for one now – you will
need it when you start studying.
A *True Certified Copy of your NSC, NC(V), Senior Certificate (if
already issued):
If you have the results but not the certificate, send
us a copy of the Statement of Results/Symbols. If you are writing NSC
in 2014, DO NOT send us your final results unless we ask you for
them.We receive them directly from the Department of Education.
A *True Certified Copy of your school examination results:
If you are still at school, we need a copy of your Final Grade 11,
June Grade 12 andTrials results (or any of these that are available)
certified by your school principal. Institutions use these results
to make selection decisions. Please ensure that your name,
surname and ID number appear on the same page as your results
(see next page for example).
A *True Certified Copy of your school leaving examination
results:
If you have written foreign or international school leaving
examinations, please send us copies of your results.
A *True Certified Copy of your academic record:
If you have
already studied at a tertiary institution.
Residence or Study Permit:
If you are not a South African
citizen and do not have a South African ID, we need a copy of any
Residence or Study Permit you have been issued.
When documents (e.g. examination results) become available after
you have submitted your application, please send to us,
clearly
marked with your CAO number.
*A TRUE Certified copy of a document is a photocopy of the original document and has
the original stamp and original signature of the Commissioner of Oaths. To speed up the
processing of your application, you are welcome to submit certified documents by email
or fax, but you will still have to send theTRUE certified copy of your ID and all results to us.

or are given by Admissions or CAO staff. If a code is not
listed, that means that the institution or campus does not
offer that particular programme.
